have a BMI of 40 or higher
have surgical weight loss coverage on your insurance plan

If your BMI is over 50 the SADI procedure could be an option, however most insurance plans do not cover this procedure.

Most surgeons would not recommend a gastric band for a patient with a BMI above 40, as it is a less powerful procedure and will typically result in significantly less weight loss in this BMI range.
