Hi Mary – gastric bypass would probably be your best option at this point. The risks of a bypass are pretty much the same as for sleeve, that is a staple line leak is the main one we worry about.

I had a Lap-Band installed April 2012 in Kenwood ‘Jewish Hospital’
I have not had any ill-effects, no erosion or any complications.
I just need a ‘fill’, but the Dr who does that at that location, requires an Upper GI before he would entertain doing this.
My insurance will not cover that, as it’s an ‘Elective’ and not a ‘Must-do’ procedure, and I cannot pay out of pocket for that.
I CAN pay out of pocket for a single visit and a fill.

Hi there, unfortunately we do require an upper GI to take a band patient on for aftercare, but we can do them right in the office which is a lot more affordable than going to a hospital.

I had gastric bypass surgery about 20 years ago. I lost about 80 lbs. but regained every bit back. I was not given much post op info for long lasting success. Was never seen by a nutritionist etc…… Do you do revisions? I understand an UGI is required. I am a retired RN due to disability. I guess it was assumed that I had knowledge for the surgery. Prior to the bypass I had plication, started at 315 now am 268. I had been taking incorrect form of vitamins all these years so I do have osteoporosis. This was an eye opener needless to say. Currently have been using bariatric shakes for meal replacement (20 gm protein content) 2 to 3 times a day and a small meal in evening. Drinking tons of water, no Soda now. No liquid with meals. Have lost 14 lbs in about a month.

There are separate Center of Excellence certifications for adults and adolescent/pediatric. We are only an adult center, so patients must be over 18. For patients under 18, Dr. Tom Inge at Cincinnati Children’s is an excellent bariatric surgery and I would highly recommend him.

I had the “Realize” Lap Band placed 01/30/2015 in Columbus, OH and lost about 55 lbs. down from 230. I am 5’3′ and 61 yrs young with no health issues. After several fills and about 9 months into the Lap Band I began to have acid reflux, night cough, and would experience vomiting after a meal. Several additional office visits and “3” XR FLUORO UPPER GI TRACT + KUB tests to date have indicated a lap band slip; no esophagus dilation. Surgeon removed all fluid in band back in October. The last UGI was performed 1/25/2017 with findings of showing a gastric lap band with a phi angle of 48 degrees. Postcontrast images reveals a phi angle of approximately 39 degrees and recommended removal. Can a slipped LB be saved? He indicated 50% of all lap band repositioning results in another slip> Needless to say I am very scared and do not want to have the band removed as it will surely result in all weight lost to be regained. Appreciated your thoughts. Thank you!

Slipped bands can be revised or replaced in many cases. In my experience, another slip is only about 10% chance. Sometimes though after a slip, even if the band anatomy is perfect, patients don’t feel the same degree of restriction as they did when their band was initially placed.

I followed him there, but he soon moved again. I have not had fills in a few years and need to start over.

I would recommend not flying for a week after a sleeve. Desk jobs usually return in 7-14 days depending on how you’re feeling.

Yes it would be a good option but would need clearance from your endocrinologist to make sure everything thyroid-related was under control first. Having said that the surgery process takes a while (sometimes 6 months with some insurance companies) so it wouldn’t hurt to get the ball rolling.

Dr. Curry- Good Afternoon! 24 years ago I had GBP w DS, had no follow up with a dietician, etc. I have severe GERD and a hernia that needs fixed. I have gained back every ounce I lost plus some. I had lapband surgery 6 years ago and lost 2 pounds!!! I am 5’4 and weigh 240. UGH ! The lapband never worked for me and the doctor that performed my surgery is no longer there. I have terrible problems with food not staying down and this occurs very often. It seems to me like I should be as skinny as a rail but that is certainly not the case. I was told that I should get the lapband out and get the gastric bypass and that it would probably be very successful. I was told I should get the bypass over the sleevectomy b/c I had the lapband already and the sleevectomy would be more risky verses the bypass. I had everything scheduled and was set to have the gastric bypass November 21st of this year but chickened out. I need a second opinion and guidance, can you help me? Also my family doctor recommended you!!!

Hello, I had a question about recovery after gastric bypass surgery. I have done considerable research and everything I read is the patient will need to stay in the hospital for 1-2 days after surgery to ensure the stomach isn’t leaking. I read on this website there is a recovery period of several hours and the patient is then sent home. I wanted to inquire is it safe to send the patient home the same day? Thank you

It’s hard to predict whether ESG is a good option without a face to face consultation, but I would say that if you are looking for around 30-50 lbs of weight loss it quite possibly may be. IF you are looking for a lot more then a more powerful procedure such as a surgical sleeve needs to be considered.

Technically balloons are not indicated for patients with prior bariatric surgery, however I have done them in patients who had Lap Bands which have been removed. I would recommend an upper endoscopy first to examine the anatomy.

I have an appointment to meet with you and do some of the other evaluations on June, 6th. I am interested in the lap band to sleeve procedure .I am self pay and hope your office can schedule my surgery mid June to around the 20th. That would work well with my schedule this summer. My BMI is 35 , I’m 54 years old and I have had a couple of cardio events and believe this surgery will extend my life. The lap band it help me to lose 30 pounds and have kept it off, but believe it or not the problem I had with it, it was very difficult to eat healthy like Brussel sprouts and other greens and fruit so I ended up eating a lot of process foods that went down easy. I went from 259 down to 198 and 2 years ago I was running half marathons, them I hurt my leg and gained 30 pounds and I just can’t seem to get the weight back off and I know the weight is hurting my heart and its time to loss it once and for all. I was banded with Dr. Sonnanstine in July of 2007. After experiencing heartburn over the last few months I finally went and had a upper GI done and was referred to Dr. Moon from my PCP. I saw Dr. Moon last week and he stated my band has slipped and needs to come out. Since my 4cc band is empty he said it would not correct itself. He will not replace the band at this time, and I think I was just so shocked about the whole thing to even ask if there was any way to fix it. I’ve read about others getting revision surgeries, and I’m assuming they have everything done at one time.
So my question is, have you’ve ever removed a patients band and replaced it with another one at the same time?
If so, I will be making an appointment to come in and see you for a second opinion.

Hi. I had GBS with a Fobi band in 12/09 and was very sick and could hardly eat and when I did, I threw up almost daily until I had the band removed and an ulcer/fistula repair surgery done in 12/13. I have not been sick since. I had a hysterectomy in 2014. Since then, I have gained 60 pounds. I feel good now…just fat. I started at 240. I am 67 and weight 340, with diabetes along with moderate neuropathy of both feet. At present my blood sugars are in control, on a 1700 cal. diet for 4 mos. but cannot seem to lose weight. Due to these problems and back pain due to degenerative disc disease and arthritis in my left hip, I have a problem doing any sort of exercise without having much pain. What would you recommend for me, I am very afraid of surgery, as I have heard many horror stories about people being ill or dying from these surgeries.

A gastric sleeve offers great long-term results with much lower risk than the old days of open gastric bypass. The risk of death is about the same as it is for gallbladder surgery. For most patients, the risk of death from an obesity related complication is going to be higher.

I had a roux en y lost 75 ,then PA put me hormone cream.I felt better but I gained the 75 plus pounds back.I stopped taking hormones, weigh gain leveled off BUT CANNOT LOSE.,which is WHY I had weightloss to begin with.I wanted the sleeve,which is what my son had.He lost 150 plus pounds NO PROBLEMS!! I have scar tissue..stomach pain..bowel issues..vomiting..nausea..wish I would have insisted on sleeve!!.WHAT ARE MY OPTIONS?? Iam miserable.sugar is up,cholesterol is up,A1C rising..pain is bad..now having to have back surgery with extra weight..HELP!!!

There aren’t a lot of great options for bypass patients who are having issues like this. Sometimes a complete reversal might help but this would need to be done at a place like Cleveland Clinic or OSU.

I have a Gastric Bypass around 2013 and my stomach and pouch has stretch and weight has been regain.I’m interested in getting the ESG to help reduce the size back down. Will medicaid cover this procedure?

Bypass patients cannot have and ESG but they can have an Overstitch procedure in some cases to reduce the size of the pouch. Medicaid does not cover either however.

I spoke to Robin yesterday regarding OverStitch. I had open RNY May of 2000 SW 298#. (There are no records except the date of surgery.) I dropped 110# when I became pregnant within a year of surgery. My LW was 165# in 2007. I maintained for 8-9 years. I am currently up and down 195-205 #. I have spoken to two surgeons who say I am a success story and no revision is required. In January of 2018 I began visiting a Weight Management Clinic near me. We tried food journeling, Saxenda, and Phentermine. The weight remained in the 195-205 range. I am consistent with 60 g protein, 60+ ounces of water, and vitamins.
Overall, I have been told I am healthy. My BMI is 38, and I have chronic joint pain specifically in the hips for which I have sought PT through a local orthopedic surgeon.
I am not looking for the “quick fix high” of the original surgery. I will be happy with working at loosing the weight. I just want to be assured the pouch and stoma have not been compromised due to the inability drop any weight.

Hi Dr Curry, I have been overweight for years. Have tried every diet in the world. Since I have gotten older it’s impossible to loose. I don’t have any serious health issues. Just arthritis in both knees. Also suffer from Depression.My guality of life is not what I would like. I know Medicare doesn’t pay for Bariatric surgery unless you have medical conditions. Am I to old to have surgery. I’m 69 yrs old. Not sure what type I should have !

I have lost weight on my own from 260lbs down to 146lbs exercising and diet. My world changed in 2012 when I found out that I have fibromyalgia. The medication has made me gain all of the weight back along with the fatigue and pain there is no way exercising is going to happen. April 2017 I found out I have Type 2 diabetes. I saw your TV commercial and thought why not! I dont have Medicaid but another insurance through my husbands job. I’m willing to try to remove the weight and the diabetes.

I had a Nissen Fundiplication 18 months ago with no LPR relief. I have not been able to find any solution including reintroduction of reflux meds and diet. Have you had success treating such patients with Stretta??

Yes we have had success, in some patients it works great and in others not so much. However for a patient who had a Nissen and still has symptoms it may be the only option.

Dr. Curry, I’m 5’8” & my weight fluctuates between 195 & 203 lbs. At one time, I was 260 lbs, dieted & lost wt., I was able to get down to 160 but gained 40 lbs back. I diet, limit my portions but my wt remains unchanged. I had a Nissan fundoplication almost 10 yrs ago. I am interested in options. I am unsure if a sleeve is an option or plastics regarding removal of loose skin around the abdomen. I am unhappy with my appearance and body image. You performed a sleeve on my spouse in Aug 2018 & he is down 80 pounds and doing well. Thank you.

Hi there and sorry for the delay. Weight loss surgery can be done successfully in patients after a Nissen fundoplication but the procedures are more complicated and have a higher risk. When a lap band erodes how long does it take for scarring and adhesions to form? What can the long term problems be after an erosion?

Howdy Teresa,
Lap Bands will cause some degree of scarring almost immediately, in most cases mild. Erosions occur over a period of years, very slowly, and the scar tissue actually helps keep gastric contents from leaking out once the erosion is full thickness. Usually everything heals up pretty quickly after the band is eroded, but in some cases there can remain a communication between the erosion and the skin where the port was, called a fistula. These cases are stubborn and can require additional procedures to correct.

Hello, Dr. Curry.
I had a lapband replaced a few years ago. Originally, I lost 100 pounds, and my band slipped. I started gaining weight regardless of what I did and did get it replaced. This time, I have not been so successful. Since that time, my insurance no longer covers weight loss surgery and I would just like to have the band removed, so I don’t have to worry about any complications. I have GERD and take Nexium every day, which helps control it. I would love to make an appointment if you would you be able to help me? I would, unfortunately, be a self-pay patient.
