Important Information For Your Initial Visit with JourneyLite Physicians

If this is your first time visiting us, or if it has been a while since you’ve been into the office, it is critical that you complete your pre-registration information online before your appointment.
You will receive a text and/or email 3 days prior to your visit with information on how to do this on your smart phone or computer.
If you are unable to complete this ahead of time, please come in 1 hour prior to your scheduled appointment time so that you can finish it in the office. If this information is not completed by your appointment time we may be forced to reschedule your appointment!
