For patients who have medical issues such as diabetes, cholesterol, or heart disease we will frequently request that your primary care physician do a pre-op physical and surgical clearance.
Most PCP’s are very accustomed to this and will know exactly what to do. They may already have a template in their electronic medical records program for doing these examinations.
Please download and print the form and letter below and take it with you to your visit with your PCP.
Sometimes your PCP may want to do some additional testing before they clear you for surgery, so it’s a good idea to schedule this appointment for around 4 weeks prior to your procedure with us.
