Diabetes, weight loss surgery, and SGLT2 inhibitors

SGLT2 Inhibitors

Diabetes management is a rapidly-changing field.  It seems like every few days there is a new medication being advertised.  If you are a diabetic, it’s important to understand how to manage your medications before and after surgery.

SGLT2 (sodium-glucose transport protein 2) inhibitors are a new class of medications used to treat type 2 diabetes.  They are also known as “gliflozins.”  If you are on a medication and the generic name of it ends in “flozin” you are likely on an SGLT2 inhibitor.  They work by preventing the reabsorption of glucose from the blood that gets filtered by your kidneys, causing excess glucose to flow out in the urine.

It is recommended that these medications be stopped 5 days prior to surgery, and not to be resumed until a reasonable amount of calories are consumed on a daily basis.  This would typically be around 4 weeks post-operatively (unless instructed otherwise).

Current medications in this class include:

  • Invokana (canagliflozin)
  • Invokamet (canagliflozin and metformin)
  • Farxiga (dapagliflozin)
  • Xigduo XR (dapagliflozin and metformin)
  • Jardiance (empagliflozin)
  • Glyxambi (empagliflozin and linagliptin)
  • Steglatro (ertugliflozin)

If you are on one of these medications, please make sure you formulate a plan with your provider at your pre-op visit!

Great news from CareSource: coverage for diabetics with BMI 30-35

We received some great news from the Ohio chapter of the American Society of Bariatric Surgery regarding weight loss surgery and CareSource. Up until now, no insurance company has covered bariatric surgery when the patient’s BMI is below 35. Now, however, CareSource has updated its policy to cover gastric sleeve and gastric bypass for patients in the 30-35 zone if they have uncontrolled diabetes. Their definition of “uncontrolled” is a hemoglobin A1C of 8 or greater.

In addition, they have added the following medical conditions as “weight-related medical issues” that can be used to potentially qualify for surgical weight loss:

  • Non-alcoholic fatty liver disease
  • Nonalcoholic steatohepatitis (NASH)
  • Pseudotumor cerebri
  • Gastroesophageal reflux disease (GERD)
  • Asthma
  • Venous stasis disease
  • Severe urinary incontinence
  • Debilitating arthritis

This is great news as it will significantly open  access to care for many CareSource patients.  We can only hope that other carriers will follow suit!

In the past, many primary care doctors and endocrinologists in Cincinnati, Columbus, and Dayton were reluctant to recommend weight loss surgery such as gastric bypass for their patients with Type 2 diabetes.

With the advent of the vertical sleeve gastrectomy (VSG), also known as the gastric sleeve, this has changed.

Recent studies such as this one published in The Journal of Clinical Endocrinology and Metabolism, show clearly that the sleeve gastrectomy can be very effective in resolving Type 2 diabetes.  Although Lap Band and gastric bypass also have been shown to positively affect this disease, the gastric sleeve offers impressive results with much less long-term risk than gastric bypass.

At JourneyLite this procedure can be performed on an outpatient basis, and patients can often be back to work in a week.

Since the AMA recently classified obesity as a disease, many patients are taking advantage of this newer procedure as a means of losing weight and improving diabetes, high cholesterol, high blood pressure, and sleep apnea.

Weight loss surgery cuts diabetes risk by 78%

Diabetes: Is It Now a Surgical Disease?

This is a great article that is narrated by the author that talks about a couple of great studies that were just published showing how effective surgical weight loss can be for curing diabetes.

Both of these studies were published in the New England Journal of Medicine, one from Cleveland (the STAMPEDE study) and one from Italy.

Both showed very impressive results for overweight or obese patients and their ability to come of their diabetes medications. Neither of the studies looked at adjustable gastric band (Lap Band / Realize Band). The STAMPEDE study looked at sleeve gastrectomy (VSG) and gastric bypass, while the Italian study looked at gastric bypass and a more aggressive small bowel bypass.

I believe that we will see more patients coming for surgery who are diabetics, and maybe we will start thinking of this as diabetes surgery instead of weight loss surgery!
