Depending on your medical history, you will need some Pre-admission Testing, or PAT’s, prior to your procedure.
If we asked you to get a pre-op history and physical from your PCP, they can do the EKG there. You can download the form below, print, and take it with you to your PCP visit. It is not required that they use it, it’s just for their convenience!
If your procedure is at JourneyLite, you will get your PAT’s done on the day of your pre-op/consent visit.
If you have cardiac issues, we may ask that you get cardiology clearance for your procedure. If we ask for cardiac clearance, your cardiologist should do your EKG. You can take this simple form to your cardiologist to expedite the process!
