It includes the surgeon fee, standard anesthesia fee, qualifying facility fee, and one year of surgery-related, office-based postoperative follow-up. Preoperative testing, outside-provider services, prescriptions, vitamins, travel, and other non-package items may be separate. Request the current written terms before scheduling.
